Activity Waiver and Release

RELEASE AND HOLD HARMLESS AGREEMENT
I, ___________________________________, for and in consideration of Close Quarter Combat, LLC and Brothers N Arms, LLC allowing me to participate in instructional activities, for and on behalf of myself and my heirs, executors, administrators, successors, and assigns, irrevocably and unconditionally, release and hold harmless Close Quarter Combat, LLC and Brothers N Arms, LLC and their owners, affiliates, insurer(s), employees, and assigns, from any and all liability for all claims, damages, losses and injuries that I might suffer or incur from all causes whatsoever as a result of participation in lessons, practice and/or presence at any other events conducted with Close Quarter Combat, LLC, and Brothers N Arms, LLC including but not limited to losses caused wholly or in part by the negligence of Close Quarter Combat, LLC and Brothers N Arms LLC, or any other persons for whose acts or omissions Close Quarter Combat, LLC and Brothers N Arms, LLC could be held liable in any manner.
I recognize that there are risks associated with the use of firearms and being in proximity of their use, and I assume all risk of injury from all such activities of Close Quarter Combat, LLC and Brothers N Arms, LLC.
Further, for and on behalf of myself and my heirs, executors, administrators, successors, and assigns, I covenant and agree not to sue Close Quarter Combat, LLC and Brothers N Arms, LLC and its owners affiliates, insurer(s), employees, and assigns for loss that arises from or results from my conduct or arises from or results while participating or attending any event sponsored by Close Quarter Combat, LLC.

PHOTO | VIDEO | AUDIO | NAME RELEASE
I hereby authorize Close Quarter Combat, LLC and Brothers N Arms, LLC to publish the photographs, videos, and audio taken of me while participating in any training classes and events, and my name, for use in Close Quarter Combat, LLC’s printed publications, Facebook sites, and website.
I acknowledge that since my participation in publications and websites produced Close Quarter Combat, LLC is voluntary, I will receive no financial compensation.
I further agree that my participation in any publication and website produced by Close Quarter Combat, LLC confers upon me no rights of ownership whatsoever. I release Close Quarter Combat, LLC and its owner and employees from liability for any claims by me or any third party in connection with my participation.

PHYSICAL CONTACT RELEASE
I understand that during training classes and on the range it may be necessary for the owner/instructors/range safety officers to touch me. The touching will be for demonstration, explanation, correction, or safety purposes and may happen on my hands, wrists, arms, areas of the back, head, legs, feet, or on the training equipment I am using.
I give permission to the owner/instructors/range safety officers to touch me as appropriate during the classes if needed.
